APPOINTMENT INFORMATION AND INSURANCE

Appointments can be made for neuropsychological consultation, testing and forensic consultations by contacting Neurocognitive Associates at 480-699-6740. Information will be taken regarding the type of service for which you are seeking care and appointment availability. We do not accept insurance at this time, but will assist you in filing information directly to your insurance carrier for reimbursement.

Insurance will generally cover testing for disorders affecting the central nervous system and psychiatric conditions that occur with a neurological or developmental disability. Insurance will not cover testing for learning disabilities unless they occur with a biologically based neurological illness. Academic and vocational testing are generally not covered by health insurance. Patients are responsible for contacting their insurance carriers to determine whether their care will be reimbursed by a non-participating or "out-of-network" provider.

Neuropsychological testing is generally covered under the medical, not mental health, portion of your insurance program and is like seeing a physician for a medical consultation. Some insurance programs allow patients to make appointments independently while some programs require that patients obtain "prior authorization or approval" from their referring doctor. Some insurance programs allow for a Point of Service option that permits patients to see the specialist of their choice who may or may not be part of your insurance program. We will provide you with the information needed for you to file your insurance.  You are responsible for filing your claim and the payment will be sent directly to you.

Insurance will generally not cover assessments for learning disabilities or vocational evaluations.
